DHI are recruiting a Transitions Worker to support clients within the South Glos area who are on one of our drug and alcohol recovery programmes. We are looking for people who can bring creativity and enthusiasm to supporting clients, helping them to make positive changes in their lives.
The transition worker's role provides individuals with the opportunity to successfully move from one stage of life or service to another. This includes a focus on care leavers or those transitioning into adulthood, alongside those moving from acute health and social care settings to community settings. As well as access to treatment services, this may involve assisting each person with housing, education, employment, and financial management. They also help individuals develop skills for independence and to navigate new services.
A Driving Licence with access to vehicle and willingness to use it for work travel is essential.
